Mites and Worms

Could someone tell me the basics of mites and worms?
A quick rummage around the site gave lots of specific questions but not the basics.
Should I be regularly mite spraying as a routine?
My birds don't go outside but I do open mesh covered windows...do mites get in or are they everywhere anyway?
Is there any certain giveaway that mites are a problem?
One of my birds is head scratching a little more than usual (he has a LOT of pins just coming through though). There are all sorts of creepy crawlies around here so mites wouldn't suprise me.
I do keep the cage clean but they do tend to trash the area.
